This course is designed to equip participants with the knowledge and practical tools needed to implement effective case management strategies across all levels of care. It begins with a theoretical foundation covering the principles, models, and benefits of case management in improving patient outcomes, reducing hospital readmissions, and lowering healthcare costs.
Following the theoretical phase, participants will engage in a practicum where they apply learned concepts within a healthcare organization. Through guided activities and real-world application, learners will develop actionable plans to support the integration of case management practices tailored to their facility’s needs.
Short Workshop
This short theoretical workshop introduces participants to the core principles of case management in healthcare. Through expert-led sessions, participants will explore strategies to improve patient outcomes, reduce hospital readmissions, and manage healthcare costs. Real-world examples and guided discussions will help attendees understand how to begin implementing case management practices across various levels of care.
What is HCCM?
Health care organizations across the world are searching for way to improve patient’s outcome, decrease readmission of patients to hospital, and to reduce healthcare cost. In this course; participants will have guidance and action steps to help their facilities to implement case management across all levels of care.
What topics will the workshop cover?
Module I: Introduction to Case Management
Covers core concepts, models, and the role of case managers, including communication, advocacy, and ethics.
Module II: Applied Case Management
Focuses on real-world scenarios, case studies, collaboration, cultural competence, and ethical decision-making.
Module III: Case Management Process
Teaches the full cycle of case management—from assessment to evaluation—with emphasis on client engagement and documentation.
Module IV: Quality & Outcomes Evaluation
Introduces tools for measuring effectiveness, setting goals, analyzing data, and driving continuous improvement.
Module V: Discharge & Utilization Management
Explores discharge planning and resource optimization to ensure efficient, high-quality care.
Module VI: Clinical Documentation Improvement
Focuses on accurate, compliant documentation using standardized language and coding practices.
Course Goal
The purpose of this course is to provide participants with required knowledge and skills to assesses, plans, implements, coordinates, monitors, and evaluates the options and services required to meet the client’s health and human service needs.
Target Participants
This course is recommended for all Healthcare Providers and Social workers with minimum Bachelor degree in medical fields with at least 2 years clinical experience.
